Age >70 years (women are at higher risk than men)
Frailty and small body frame
Multisystem disease (for example chronic renal insufficiency, heart failure)
Hypothyroidism
Solid organ transplant recipients
Polypharmacy
Perioperative period
Specific concomitant medications or substances, such as:
Fibrates (especially gemfibrosil)
Nicotinic acid (rarely)
Ciclosporin
Azole antifungals (itraconazole and ketoconazole)
Macrolide antibiotics (erythromycin and clarithromycin)
Protease inhibitors (for HIV infection)
Nefazodone
Verapamil
Amiodarone
Large quantities of grapefruit juice (usually more than 1 quart (1.14 l) per day)
Alcohol abuse
